During this consultation the Doctor will discuss nutritional habits, create nutritional plans, and discuss the potential for quality supplementation.

Please call to schedule a Nutritional Counseling appointment with the Doctor, and bring the following:

• List of any prescription or over the counter medication you currently use (include dosages, prescribing Doctor, condition, and dates)
• List of any supplementation you currently use (Include brand, dosage, length and reason for use)
• A “Food Journal” for three full, recent, average days (include every food or beverage that you put in your mouth, dates/time of intake)
